Welcome to the Sleep Coach. I'm your host, Shelly Simpson. If you're exhausted, but your mind won't shut off at night, you're in the right place. Each week, we'll unpack the science of sleep, hormones, and the nervous system so you can understand what's happening in your body and finally get the rest you deserve. Here we quiet the 3 a.m. mind, calm the body, and help you finally get the sleep you've been missing. If you ever laid in bed completely exhausted while your brain replays conversations, plans tomorrow, or spirals into anxiety at 2 a.m., this episode is going to help you understand exactly what's happening inside your brain and nervous system during midlife and why insomnia becomes so common for women over 40. First, midlife insomnia is real. It's highly physiological. And I want to normalize this because so many women blame themselves for their sleep struggles. Research shows that nearly half of women in perimenopause and menopause experience insomnia symptoms. And for many women, it becomes chronic. This isn't simply just getting older. It's not a lack of discipline, willpower, or better bedtime routine. What's happening is that several systems in the body that regulate sleep become more sensitive and more dysregulated during midlife. For instance, the brain, the nervous system, hormones, stress response pathways, and even blood sugar regulation. And all of those systems communicate with each other constantly. So when women say, I'm exhausted, but my brain won't turn off, that's not imaginary. There are very real neurological and hormonal reasons why this happens. Your brain was designed to keep you alive, not necessarily asleep. So one of the biggest mindset shifts in understanding that your brain's primary job is survival. Sleep only happens when the brain perceives enough safety to allow the body to power down. And several parts of the brain are involved in that process. So let's first talk about the thinking brain, which is the prefrontal cortex. And it sits right behind the forehead, and it's responsible for planning, decision making, problem solving, self-control, and future thinking. It's basically your executive center. During the day, this part of the brain helps you manage life, kids, schedules, work, relationships, responsibilities. But at night, when all those external stimulations decrease, unresolved thoughts often become more noticeable. And that's why many women suddenly start mentally reviewing tomorrow's schedule or replaying conversations when they lie down. Yes, that's real. That rumination of why did that person say that to me, or I should have said this to them, or blah, blah, blah. You know how that goes. Just replaying that conversation that you wish you would have had, or maybe you wish you wouldn't have had. The brain is attempting to complete unfinished cognitive loops before it sleeps. So the next part of your brain that we're going to talk about is the amygdala, and it's basically your threat detector. The amygdala is part of the brain's emotional processing system and it acts like an internal alarm system. Its job is constantly to ask: are we safe? When stress levels are chronically high, whether emotional stress, inflammation, poor sleep, overwork, or hormone fluctuations, the amygdala becomes more reactive. This is important because estrogen helps regulate amygdala activity. As estrogen fluctuates during menopause, many women experience increased anxiety, heightened stress sensitivity, nighttime rumination, and hypervigilance. So the brain becomes more likely to interpret ordinary thoughts as urgent or threatening at night. Now let's talk about the stress response system, which is your HPA axis. This next piece is huge when it comes to insomnia. Your brain and adrenal glands, which your adrenal gland is a gland that sits right on top of your kidneys, communicate through something called the HPA axis, and that's your hypothalamic pituitary adrenal axis. It's very important in your menopause, perimenopause, and menopause to keep this HPA axis in balance. The system regulates your stress response and controls cortisol production. Under normal circumstances, cortisol should be low at night and then it rises gradually in the early morning and helps you wake up naturally. But chronic stress changes this rhythm. In women who are under prolonged stress, physical or emotional, cortisol patterns often become dysregulated. Instead of dropping low at night, cortisol may spike around 2 or 3 a.m. And cortisol is activating. It increases alertness, raises your heart rate, mobilizes glucose, and signals the brain to do what? Wake up. Progesterone is declining in perimenopause and menopause. And progesterone is gives you that calming effect. It's not a hormone that's talked about that much, but it is starting to hit the mainstream. I'm starting to hear more about progesterone on like other podcasts and books that I've read and stuff. So it is starting to hit the mainstream on what this particular hormone does. It has this calming, sedating effect on the brain because it interacts with GABA receptors. GABA is the brain's primary calming neurotransmitter. You can think of GABA as the nervous system's brake pedal. When progesterone declines during perimenopause, women often become more stress-sensitive, sleep becomes lighter, anxiety increases. And this is one reason women often say, I suddenly became a light sleeper in my 40s when I never used to be. Okay, now let's talk about the nervous system and why your body won't power down. This is because insomnia is not just happening in the brain, but the nervous system is also involved. Your autonomic nervous system has two major branches, and we've talked about this in a previous episode: the sympathetic nervous system and the parasympathetic nervous system. The sympathetic nervous system is your fight or flight response. It's that alertness, that action, that stress, the survival that, you know, a bear is chasing me. I have to hurry up and run mode. The parasympathetic system is your rest and digest state. That's recovery, healing, relaxation, sleep. This is where we want to be parasympathetic. The problem is that many women are spending most of their lives stuck in sympathetic dominance. That constant productivity, that constant mental load, constant stimulation. You're just your nervous system can't instantly switch from go, go, go mode to okay, now let's sleep deeply. And why this feels worse at night is that all those external distractions start to disappear at night. When the body finally becomes still, the nervous system finally has enough quiet to notice everything it has been suppressing all day. That's why nighttime thoughts often feel louder, more emotional, and even more catastrophic. It's not a weakness, it's nervous system activation combined with reduced cognitive distraction. Now, let's talk about why forcing sleep backfires. Here's where many women accidentally worsen insomnia. They fight wakefulness. They start fighting wakefulness. The moment the brain perceives pressure, I have to sleep. And stress activates alertness pathways. This is why clock watching, frustration, overthinking, doom scrolling, and mentally calculating how little sleep you've gotten or that you're gonna get. Ever been there? You just watch the clock and you're like, oh my gosh, I'm I've only been asleep two hours and I have to get up in one hour. Oh, you ever done that? Yeah. I'm with you. All reinforce, all those things reinforce wakefulness neurologically. Now, what actually happens physiologically and what helps? You have to stop treating insomnia as purely a thinking problem and start supporting the nervous system and brain physiology. That means stabilizing stress responses, breathing activities to help reduce that stress. Calming the body first, regulating your blood sugar, making sure you've had enough to eat during the day. If you're in that constant go mode, you may not even realize that you skipped breakfast, lunch, and now it here it is at eight o'clock at night, and you're like, oh my gosh, I really haven't hardly ate anything all day. So you gotta make sure you're fueling yourself all day. So regulating your blood sugar, reducing nervous system overstimulation, just constant being constantly on your screens or just, you know, being in meetings all day long, uh, being in, you know, that that type of mode is that's overstimulation. And then you come home and you're on your phone scrolling all night. That that's your brain doesn't have time to power down. So, and then also supporting your circadian rhythm. One thing you can do to support that is get morning sunlight. So it's sometime, you know, after you wake up and you know, maybe before, well, yeah, preferably before 9 a.m., if you can at all possible get direct sunlight. If you're able to get up to maybe 10 minutes in the morning of direct sunlight, no sunglasses. Obviously, you don't want to look straight in the sun, but you need direct sunlight on your retinas to help support that circadian rhythm and help get it back to where it should be, awake in the morning, awake during the day, and then starting to power down at night. Then all of this just creates cues of safety for the brain because sleep is not something you force, sleep is something the brain allows when the body feels safe enough. There's reassurance and there's hope. The good news is that the brain is adaptable, your nervous system can change, stress pathways can calm down, sleep patterns can improve. And when women understand the physiology behind what's happening, they stop blaming themselves and start supporting their bodies differently. And that's where real healing begins.
